Draft the appeal the desk does not write. The coordinator still files it.
The DeskGate Operator Extension (v2.6.8) docks in the coordinator's already signed-in Chrome or Edge session. It drafts the appeal or non-API prior authorization from quoted chart facts, writes [MISS] where the chart is silent, and types into empty portal boxes. The coordinator reviews, attaches files and clicks Submit. We license by named portal family, not by case. First 10 desks: 30 days free.
First 10desks get 30 days free on one named portal family
1–3 claimsper month to break even · S-Practice at $490/mo, S-Desk at $1,950/mo
Zerocase metering on any plan, and no share of recovered revenue
Step 1 · Payer portal lane planner
Count the portal families, not the cases.
A lane is one named payer portal family, such as Availity, CoverMyMeds, a dental carrier portal or a state Medicaid portal. Move the slider to the number of families your desk works and the plan follows.
123456
One family might be Availity or CoverMyMeds alone ($490/mo for 1–3 seats on S-Practice, or $1,950/mo for unlimited seats on S-Desk). Two or three families map to S-Agency ($4,900/mo). Zero case counting on every tier.
1 to 3 claims / moZero case metering · no revenue share
The first 10 desks start with 30 days free on one named portal family. After day 30, stop or subscribe.
Step 2 · Plans (Schedule S)
Start free for 30 days. Break even on 1 to 3 claims a month.
Every plan starts with the 30-day free measurement for the first 10 desks. Pay monthly or prepay for the year. There is no case metering, no share of recovered revenue and no required 12-month commitment.

On every plan, including the free month: appeal and prior-authorization drafts that quote facts already on screen, [MISS] where the chart is silent, typing into empty portal boxes, and a human Submit. Screen reads run on Vertex AI under DeskGate's Google Cloud BAA. Desks bring no API keys.

The first 10 desks
Breaking the reference catch-22.
Healthcare software procurement stalls on a catch-22: clinics wait for reference customers, and vendors wait for paying clinics. We break it by covering the first 10 desks for 30 days, at our cost.
1
No fee for 30 days
No subscription or setup invoice. Software access, lane mapping and the 30-day Schedule M measurement are at our cost, in exchange for one coordinator, a pre-period baseline and a blinded throughput ledger DeskGate may publish.
2
Your hardest portal family first
We start on the payer portal family your desk already works hardest, such as Availity, CoverMyMeds or a state Medicaid portal. We do not promise a minute count in advance.
3
A 30-day ledger
On day 30 you get the Schedule M ledger: cases completed per coordinator per day against your pre-period baseline, with no overturn rate claimed. The coordinator remains the filing actor throughout.
Sign-off sits with the practice manager, clinic owner, or billing director. Before a live chart is read, the desk signs its own BAA with DeskGate.
Healthcare funds and MSO sponsors. Evaluating a portfolio-wide rollout, or diligence on the pre-seed SAFE?
DeskGate does not monitor coordinators, inspect case queues or write patient records to its databases. The license is tied to portal hostnames, not to case counts.
One check when the extension loads
When the extension or embed loads, it asks one question: is this organization entitled to this portal hostname? If yes, staging turns on. If no, it stays off. That is ordinary enterprise licensing by system capability.

The 30-day Schedule M ledger
BAA executed 28 Sep 2026 · deskgate-production
Where screen reads go, and what is not stored
Screen reads go to Gemini on Vertex AI in project deskgate-production, under the Google Cloud BAA executed 28 September 2026. Chart notes and member IDs are not written to DeskGate databases, and the pilot ledger stores a SHA-256 of the payer reference, not the reference. The BAA's terms govern Google's side (Master Terms §4).
Before a live chart is read, the desk signs its own BAA with DeskGate (or Schedule CH AVV in Switzerland).
Risk split
Execution is ours. The workflow is yours.
We do not underwrite a desk's cohort, portal family or exception rate. Each side answers for what it controls, which keeps the agreement grounded.
DeskGate is responsible for execution fidelity under the product rules
Working inside the coordinator's own signed-in session
Staging text only into empty portal fields
The human gate: nothing is submitted without a person clicking Submit
Hashing the payer reference in the browser before it reaches the ledger
The desk is responsible for workflow parameters
The target cohort for the 30-day measurement
Which cases are exceptions
The people who review each draft, attach files and click Submit
